The Intercollegiate General Surgical examination is at a crossroads. It has now been broken down to its component parts and each has been subjected to intense educational review in order to meet the specifications of PMETB. As a result in November the first test of knowledge will be introduced, this will consist of an MCQ in General Surgery. When this hurdle has been passed the candidate will some months later complete the examination in General Surgery by a Clinical examination consisting of an Academic paper, clinical scenarios and a standard clinical examination involving patients.

Considerable controversy still exists regarding the Subspecialty component and the option appraisal of each will be discussed and will be related to the latest version of the Modernising Medical careers document. Work place assessment and exit examinations will be considered and the evidence base for each will be discussed. The future direction of the examination and the Colorectal component is still controversial however the options are evident and depend on the funding of Subspecialty training and an agreement about the Subspecialty requirements over the country by the government.
